Better spam protection

Description
After recent spam, some changes were done. However, they are not enough:
- CAPTCHA: not secure against targeted attacks
- Two posts required: it is possible to do them easily
- Two days required: spammers will register many accounts and start activity after two days

this sort of thing is not like a normal feature - it is more of
an evolving process, that should be of no concern to users - it
is entirely extra workload upon the admins with zero benefit to
anyone, managing some balance of erecting hurdles, explaining
why they are there, and cleaning up what falls through the cracks
the only consideration is to minimize the amount of developer time
that is wasted by these useless chores
